For which conditions is ketamine therapy effective?
Ketamine is an anaesthetic that has been known for decades and is used for short anaesthesia during minor surgical procedures or in emergency medicine. It is not only used to treat depression, but also post-traumatic stress disorder (PTSD), anxiety disorders, addictions and obsessive-compulsive disorder (OCD). How does ketamine work?
Unlike other antidepressants, intravenously administered ketamine takes effect immediately. The effect usually occurs during the first infusion and typically reaches its maximum the following day. However, the duration of the effect varies: sometimes only a few days, often weeks, and in rare cases even months or years. For this reason, several infusions are usually necessary or – even better – a combination with other methods such as psychotherapy, hypnosis or magnetic stimulation. Ketamine can be a breakthrough, especially for people who have had many unsuccessful attempts at therapy with a wide variety of antidepressants. Suicidal thoughts in particular disappear. Ketamine is currently the only known anti-suicidal substance.
The majority of patients report a significantly improved mood and clearer thinking immediately after the infusion. This effect is usually even more pronounced the following day, even though the substance is no longer detectable in the body due to its extremely short half-life. This effect lasts for days to months, sometimes even years.

Ketamine-assisted psychotherapy takes advantage of the fact that ketamine improves neuroplasticity. This makes patients more open to psychotherapy and neurofeedback. This creates an important window of opportunity in which intensive therapeutic work can be carried out with the patient with the aim of achieving lasting change. In severe cases of depression in particular, the ability to learn is reduced, making psychotherapy and neurofeedback therapy more difficult. We also successfully use combined ketamine therapy for treatment-resistant depression.
Ketamine therapy can also be used for postpartum depression, as the substance is metabolised very quickly in the body and the treatment is therefore also suitable for breastfeeding mothers.

Procedure for ketamine infusion for depression
Treatment always begins with a detailed preliminary consultation. This allows us to gain an understanding of your personal situation. The results of previous treatment attempts, your medication intake and any existing or past illnesses are also important.
The detailed medical history is supplemented by psychological testing and a quantitative EEG, which provides information about brain function. In addition, a medical examination is carried out, including blood pressure measurement and, if necessary, an ECG and HRV measurement. Depending on the symptoms and issues, a mobile sleep laboratory is set up for the first night.
In ketamine therapy for depression, the drug is administered in a very low dose as an infusion over approximately 40 minutes. This allows the ketamine to exert its antidepressant effect. Side effects are almost never observed, and if they do occur, they disappear immediately after the infusion is stopped. However, patients sometimes experience a feeling of warmth and feel slightly intoxicated. Of course, a thorough examination is carried out before the infusion so that precautions can be taken for certain patients (high blood pressure, cardiac arrhythmia, psychosis). However, administration as an infusion under supervision is apparently too costly for many practices.

Ketamine therapy is particularly effective for depression caused by chronic pain or depression that manifests itself primarily as pain (somatised depression). Ketamine therapy can also be effective against anxiety itself.

Medical studies on ketamine
Gradually, more and more studies are being published on the effects of ketamine on various clinical pictures: for example, it has been proven that ketamine therapy is also very effective in treating post-traumatic stress disorder. In addition, ketamine therapy appears to reduce the urge to consume drugs again in the long term in cases of opiate addiction and other addictions.
In the USA, a fast-track procedure is underway to approve ketamine infusions for depression. But even in Europe, the use of ketamine as an off-label therapy is not a problem: since the drug is approved, it only requires that the patient be given detailed information. Off-label therapies are completely normal and common in many areas of medicine.
Recently, many studies have also confirmed the effectiveness of ketamine infusions for obsessive-compulsive disorder. For many people, the treatment leads to a significant reduction or disappearance of symptoms, sometimes over several weeks after a single infusion. This time can then be used for intensive psychotherapy, often making long-term pharmacological therapy unnecessary.
