Major Depression is one of the biggest challenges for the health systems in the next decades (WHO). Nearly 20 percent of the population suffer at least once in their life from a depressive episode what sometimes leads to a suicidal action.

Conclusions:
Adjunctive ketamine demonstrated a greater reduction in clinically significant suicidal ideation in depressed patients within 24 hours compared with midazolam, partially independently of antidepressant effect.

Ketamine has been a well-known anesthetics over many decades, which can be used for short anaesthesia during minor surgical procedures or in the emergency medicine. It was noticeable for a long time that it led to an increased dream activity. For this reason it spread also in the drug scene.
This story seems to impede the dissemination of this safe and with few side effects medication.A lot of psychiatrists avoid the use ofnarcotics, others feel repelled by the image of the medicament as a “drug”. A psychiatrist said to me that we would replace one evil by another.

Posttraumatic stress disorders result from stresses of exceptional magnitude or catastrophic extent. There is a sense of helplessness and a shattering of the understanding of the ego and the world. Sleep disturbances, increased vegetative tension, concentration disorders, affectability, depression, anxiety and avoidance behavior follow. Many patients withdraw socially, drug abuse increase, along with suicidality. Chronic pain syndromes are often the result of a posttraumatic stress disorder.

After the tragic Germanwing crash due to a mental disorder of the co-pilot, many commercial pilots are afraid to disclose their own possible mental health issues or psychosocial crisis situations, because they fear the loss of their pilot’s licence. The disturbing conclusion: depression remains untreated.
Psychiatric disorders and/or depression are found in pilots just as often as they can be found in the general population. However, for pilots, such a disease may lead to the loss of theirprofessional licence and constitute, therefore, athreat to their livelihoods. This makespilots and crew members become secretive about their psychological problems, allowing it to become ataboo within the profession,adding a supplementary burden to a profession that alreadybears great responsibility and an extremely stressful workload.
